先日お知らせしたモデルナ製ワクチンの異物混入による一部使用停止に伴う本学接種分の再納品について
本日8月30日、再納品が予定通り行われることが通知されました。
本学の職域接種につきましては、予定通り9月4日(土)から行います。

報道等にありますように、モデルナ製ワクチンの一部に異物が混入しているのが見つかり、同じ工程で製造されたワクチンの使用見合わせが決まっています。確認したところ今回の職域接種で使用するワクチンについては、対象となるロット番号になっています。
現在、厚生労働省はじめ関係機関と確認し、ワクチン再納品の日程を確認しています。9月3日(金)までに再納品されれば、9月4日(土)からの接種は可能ですので現時点では予定しているスケジュールで職域接種を実施する予定です。
再納品日程によっては、スケジュールを変更する可能性もございます。 最新情報が入り次第改めてこちらのページでお知らせします。

【実施スケジュール】
■接種実施期間：
1回目：9月4日(土)～9月5日(日) 9:00～16:00
2回目：10月2日(土)～10月3日(日) 9:00～16:00
※必ず2回セットで接種をお願いいたします。（1回目と2回目が別会場は不可）
※2回目の接種スケジュールについては、ワクチンの納品等の関係で変更になる可能性があります。
■予約受付期間：
8月25日(水) 10:00 ～ 9月1日(水) 12:00 (正午)
※上記期間内に、本予約サイトにてご予約いただけます。

【接種日予約について】
■1回目と2回目のセットでのみ予約が行えます
1回目、2回目の接種日のスケジュールをご確認のうえ、ご予約ください
※2回目の接種日は、1回目の接種日から4週間後の同じ曜日・同じ時間となります
※必ず2回セットで接種をお願いいたします。（1回目と2回目が別会場は不可）
■自治体での接種や他の職域接種との二重予約とならないようご注意ください
■STORESへの新規登録やログインを行う必要はありません
■予約登録が完了すると、入力したメールアドレスに予約の確定メールが自動送信されます。
メールが届かない場合は、【お問い合わせ】よりお問い合わせください。
【予約の取消・変更について】
（1）予約の取消：予約の確定メール内『キャンセル・変更はこちら』より、お取消しください
（2）予約の変更：（1）で予約の取消しを行ったのち、新たに予約をお願いします
※予約期限（9/1の12:00）を過ぎての予約取消・変更を行うことはできません。
【接種当日の持参物】
①本人であることが証明できるもの（健康保険証・マイナンバーカード・運転免許証・パスポートなど）をお持ちください。
あわせて学生・専任勤務員は学生証・職員証など、所属が証明できるものをお持ちください。
②予診票：大学の学内ポータルサイトにログインしてダウンロードしてください。
※必ず住民票記載の住所をご記入ください
※『診察前の体温』は当日自宅で検温し、ご記入ください
※『新型コロナワクチン接種希望者』の欄の自署をお忘れなくご記入ください
③接種券（自治体から届いている方）もしくは接種記録書（接種券が届いていない方）
※接種記録書は大学の学内ポータルサイトにログインしてダウンロードしてください。
※自治体からの接種券は届いているが、接種日当日に持参することができない方は、
接種記録書をご持参ください。
当日は必ず不織布マスクをご着用ください。
接種当日は筋肉注射になるため、接種当日は、袖を完全に上げることのできる衣服で
お越しください(写真参照、肩を出せる服装)。

■Vaccination Schedule
First dose: Saturday, September 4 and Sunday, September 5, 2021, 9 a.m. to 4 p.m.

■You can only make a reservation for the first and second doses together.
Please make sure your schedule for two doses before making reservation.
* The second dose should be reserved on the same day and time four weeks after the first dose.

[Cancellation or change of your reservation]
(1) Cancellation: Access “Cancel and Change of your reservation” in the confirmation email and cancel it.

You must wear a mask (non-woven mask is recommended) on the day of vaccination.
The vaccine is injected into the upper arm muscle. Please wear clothes that make it easy to access your shoulders (see below).
